A digital signature is fine, or you can print, sign, and send a photo or scan of the form.

 

Email the completed form to info@tgocondo2.org, or drop off at the TGO CSA Office.

 

According to Florida law, our Quarterly and Special Meeting Notices require a signed consent form on file for an email notice to be considered "official required notice." Without this form, the law requires us to send you physical mail every time we have a meeting notice.
